Weight loss medication for over 50s

Weight management after 50 is both more challenging and more important. Age-related muscle loss, slowing metabolism, and accumulated health conditions make losing weight harder — but the health benefits of weight loss become even more significant at this age.

Why weight matters more after 50

Excess weight over 50 dramatically increases your risk of:

  • Cardiovascular disease: The #1 cause of death in Australia. Risk accelerates with age and weight.
  • Type 2 diabetes: Insulin resistance increases with age and visceral fat.
  • Osteoarthritis: Every 1kg of weight puts 4kg of stress on your knees. Joint problems can reduce mobility and quality of life.
  • Certain cancers: Obesity is linked to at least 13 types of cancer, with risk increasing after 50.
  • Sleep apnoea: Prevalence increases with age and weight.

Losing even 5–10% of body weight can significantly reduce all of these risks.

Age-specific medication considerations

Best options for over 50s

  • Wegovy: The top choice for this age group. The cardiovascular risk reduction (SELECT trial) is especially relevant for over-50s. If you have established heart disease and BMI ≥35, you may qualify for PBS pricing when listed.
  • Mounjaro: Maximum weight loss (~21%). Safe for over-50s. Good choice if you need to lose a substantial amount.
  • Xenical: The safest option with the fewest systemic side effects. Minimal drug interactions. Good for patients on multiple medications.

Use with caution

  • Duromine: The stimulant effects (elevated heart rate, blood pressure) pose higher cardiovascular risks in over-50s. Many doctors will not prescribe it to patients over 65.
  • Contrave: Generally safe but blood pressure should be monitored closely.

Critical: muscle preservation

After 50, you're already losing muscle mass at ~1–2% per year (sarcopenia). Rapid weight loss from medication can accelerate this. Muscle loss reduces metabolic rate, impairs mobility, increases fall risk, and reduces bone density.

Non-negotiable for over-50s on weight loss medication:

  • Resistance training: 2–3 sessions per week. This is the single most important intervention alongside medication.
  • Protein intake: 1.2–1.6g per kg body weight daily. Higher than general recommendations because protein needs increase with age.
  • Monitoring: Regular DEXA scans or body composition measurements to track muscle vs fat changes.

Kidney function monitoring

GLP-1 medications are processed by the kidneys. Kidney function naturally declines with age. Your doctor should check kidney function (eGFR and creatinine) before starting and periodically during treatment. Dehydration from GI side effects can temporarily worsen kidney function — stay very well hydrated.
