Why dose escalation?
Starting at the full therapeutic dose would cause severe nausea and vomiting in most patients. Gradual escalation allows your GI system to adapt, making the medication tolerable. Most side effects occur during dose increases and improve within 1–2 weeks at each level.

What if side effects are too bad?
If you experience severe nausea or vomiting at a dose increase, your doctor can:
- Extend the current dose: Stay at the current level for an extra 2–4 weeks before increasing
- Prescribe anti-nausea medication: Ondansetron (Zofran) can help manage severe nausea
- Adjust the schedule: Some patients do better with smaller dose jumps (where the pen allows)
- Accept a lower maintenance dose: Not everyone needs the maximum dose — significant weight loss can occur at lower doses
Cost during escalation
One benefit of escalation: your medication costs less in the first few months because you're using lower (cheaper) doses. This reduces the first-year total cost compared to annual maintenance pricing.
