1. Mounjaro — best for maximum weight loss
Technically a dual GLP-1/GIP agonist (not pure GLP-1), Mounjaro produces the highest average weight loss at ~21%. Its dual mechanism appears to provide superior fat metabolism and energy expenditure. Lower nausea rates than Wegovy (~29% vs ~44%). The downside: no PBS prospect and less long-term safety data than semaglutide.
2. Wegovy — best overall (evidence + CV benefits + PBS potential)
The most-studied weight loss medication with 15,000+ trial participants and 4-year follow-up data. The SELECT trial proved a 20% reduction in cardiovascular events. PBS listing is pending, which could make it dramatically cheaper for eligible patients. Produces 15–17% average weight loss.
3. Ozempic — best for type 2 diabetes (but restricted for weight loss)
Contains the same ingredient as Wegovy at a lower dose. Still available and PBS-subsidised for diabetes (~$41/script), but restricted for new weight loss prescriptions since October 2024.
The bottom line
For weight loss in 2026, your real choice is between Mounjaro and Wegovy. Both are excellent — Mounjaro wins on pure effectiveness, Wegovy wins on evidence depth and PBS potential.
